New England Cancer Specialists (NECS) is currently seeking a
full-time 40hr./Monday-Friday Nurse Navigator to join our new
Survivorship/High Risk Oncology Program. This is an exciting
opportunity for an enthusiastic oncology nurse with
experience/interest in genetics and holistic health. This position
is eligible for up to a sign-on bonus paid after 90 days of
employment.
This unique position will provide direct care to two very
specific populations, patients who are at high risk for developing
cancer based on family histories and genetic mutations and patients
who are in the survivorship stage of their cancer journey and are
looking to take a more holistic approach to their continued health
needs.
Essential Duties with the High-Risk Clinic

Taking detailed medical histories of patients

Evaluating patients for the presence of genetic and
non-genetic risk factors of disease

Gathering a detailed family history of patients

Teaching patients and their family about their condition

Interpreting lab data
-
Creating patient care plans
-
Coordinating patient care needs with other interdisciplinary
healthcare team members

Facilitating genetic testing
-
Providing emotional support to patients and their loved ones/caregivers.
Essential Duties for Survivorship Clinic

Assisting patients to create and maintain routines that can
help them enhance and maintain their overall wellness.

Help patients see their health from a body, mind, and soul perspective.

General Job Duties:

Point of contact for referring physicians,
patients, and caregivers to assist with patient scheduling and
coordination of care.

Assist physicians; conduct patient assessments.

Maintain communication with patients, families, and the health
care providers to monitor patient satisfaction with the
cancer/blood disorder experience.

Receive patient telephone calls and intervene appropriately
for that patient’s specific needs in a timely manner. Alert
physician as needed to patient needs that require physician
intervention. Alert primary oncology team if indicated.

Maintains patient care standards and assures that patient care
functions, such as assessment, treatment and documentation are
completed safely and effectively.

Coordinates patient care responsibilities within the scope of
the practice for licensed and unlicensed health care providers.

Community Outreach: Builds partnerships with local agencies
and groups (referrals to other services and/or survivor support groups).

Demonstrates respect for life, dignity, and rights of all persons.
**Current and former NECS employees are not eligible for sign-on
